Elke staat heeft weer aparte regelingen. Massachusetts kent bijv. de meest vergaande Medicaid regeling en heeft sinds 2006/2007 een ziektekostenregeling voor iedereen (universal coverage). Op 23 maart 2010 is de de Health Care Reform Billondertekend waardoor er de komende 10 jaar o.a. 16 miljoen meer Amerikanen met weinig inkomen onder de Medicaid ziekenfondswet vallen. Medicare is een uit 1965 daterend social program voor alle 65-plussers (in 2010 waren dat 38 miljoen senior citizens). De Medicare-website heeft ook een lijst met definities (link) van veelvoorkomende termen in de Amerikaanse gezondheidszorg. Verder is er ook nog het Children’s Health Insurance Program (CHIP). (AmE) intern: “In the US and Canada (on occasion elsewhere) the term (sometimes spelt interne) for a recent medical graduate, resident and working under supervision in a hospital as part of his or her training. The equivalent terms in Britain are houseman, house physician, and house surgeon.” (Fowler’s, 2004)

(AmE) house officer (house staff) = een overkoepelende term waarmee zowel (AmE) intern als (AmE) resident kan worden aangeduid

In zijn roman Five Patients (1979, published in Arrow Books in 1995), die de inspiratie vormde voor de Amerikaanse TV-serie ER, geeft Michael Crichton de volgende definities:
An intern is thus anyone with an M.D. [= Doctor of Medicine, a 2nd degree] who is in his first year out of medical school. An intern is licensed to practice only within the hospital. After a year of internship, he could theoretically leave and begin private practice, but practically nobody does. Instead interns go on to become residents. (Crichton, Five Patients, Arrow Books, p. 172)
A resident is anyone who has finished his internship and is continuing with more specialized training such as pediatrics, surgery, internal medicine, or psychiatry. A residency may be taken at the same hospital as the internship or at another; residencies last from two to six years, depending on the field. (Crichton, Five Patients, Arrow Books, pp. 172-3)
Interns and residents (…) are hospital employees and are referred to as “house officers”. Collectively the interns and residents comprise the “house staff”, as distinct from the “senior staff”,  meaning the private physicians or academic teachers affiliated with the hospital. (Crichton, Five Patients, Arrow Books, p. 173)

De Nederlandse term “arts in opleiding tot specialist” (aios) dateert van januari 2005. Het is een verzamelterm voor de tot 2005 gebruikte termen “assistent-geneeskundige in opleiding tot medisch specialist”(agio), huisarts in opleiding (haio), en verpleeghuisarts in opleiding (vaio). Zie wikipedia link.
De periode als (BrE) specialty registrar voorafgaand aan een ziekenhuisspecialisme is minimaal 6 jaar; en voorafgaand aan de functie van huisarts (GP) 3 jaar. Zie wikipedia link.

In de in 2008 gepubliceerde roman The Private Patient besteedt P.D. James uitvoerig aandacht aan de nieuwe Britse terminologie (in het kader van het nieuwe systeem genaamd Modernising New Careers), en plaatst daarbij kritische kanttekeningen: “This New MMC – Modernising Medical Careers – makes training schemes far more rigid. Housemen have become foundation-year doctors – and we all know what a mess the government have made there – senior house officers are out, registrars are specialist surgical trainees, and God knows how long all this will last before they think of something else.” (book 1, chapter 10)

Sinds 2007 is de Britse terminologie is als volgt veranderd:
– (BrE tot 2007): house officer (= pre-registration house officer = PRHO) = co-assistent / medisch stagiair in een ziekenhuis tijdens 1e propedeusejaar (foundation year 1). Ook wel BrE, tot 2007: houseman
– (BrE tot 2007): senior house officer = co-assistent / medisch stagiair tijdens 2e propedeusejaar (foundation year 2): tijdens deze periode wordt een keuze gemaakt tussen een verdere opleiding tot huisarts (GP) en een opleiding tot medisch specialist.
(BrE vanaf 2007): foundation house officer (gedurende twee propedeusejaren) = foundation-year doctor (zie hieronder, P.D. James)

(Ned) medisch stagiair bij huisarts
(BrE per 2007): Specialty Registrar (= StR): gedurende 3 jaar voor de huisartsenopleiding, en 6 jaar voor de opleiding tot specialist.

In het geval van Britse medische specialisten wordt de aanspreektitel Dr (“Doctor”) soms vervangen door Mr, Mrs, Ms, Miss. Dit geldt voor allen die Member of the Royal College of Surgeons (MRCS) zijn. Deze titel (Mr/Mrs/etc.) heeft een hogere status dan Dr. “Mister is one above Doctor” (uit de Britse ziekenhuisserie West Wing).

When a medical doctor passes the examinations which enable them to become a Member (or Fellow) of one or more of the Royal Surgical Colleges (“MRCS”), it is customary for them to drop the “Doctor” prefix and take up “Miss”, “Mister”, or etc. This rule applies to any doctor of any grade who has passed the appropriate exams, and is not the exclusive province of consultant-level surgeons. In recent times, other surgically-orientated specialists, such as gynaecologists, have also adopted these prefixes.
Vandaar dat P.D. James in haar roman The Private Patient (2008) opmerkt dat niet alleen het personage  van de plastisch chirurg Mr Chandler-Powell, die zijn hoofdpraktijk heeft in het Londense Harley Street, maar ook zijn assistent Mr Marcus Westhall de titel “Mr” mag voeren: “he’s a surgeon so they call him Mr” (book 2, chapter 6)

de letterlijke betekenis van farsighted komt alleen in AmE voor (BrE: long-sighted);
de figuurlijke betekenis van farsighted ( “met vooruitziende blik”) is zowel BrE als AmE;

(BrE) ” nudism” is een minder specifieke term: naaktrecreatie.
Volgens wikipedia (link) worden in het Amerikaans-Engels de termen nudism/nudist en naturism/naturist als hoegenaamde synoniemen gebruikt voor BrE “nudism” (= Ned. nudisme) :
The usage and definition of these terms varies geographically and historically. Though in the United States, naturism and nudism have very similar meanings, in Britain there is a clear distinction.

(BrE) pharmacies, zoals Boots, lijken vaak op de (AmE) pharmacies.
In de (BrE) pharmacy bevindt zich een apotheek (chemist’s), maar daarnaast verkoopt men er ook verzorgingsartikelen, toiletartikelen, make-up, en het vervult preventieve medische handelingen zoals zwangerschapscontroles, bloedtests, gezondheidsadviezen, etc.”

(BrE) pharmacy(BrE) chemist's

(AmE) pharmacy heeft een veel ruimere betekenis dan een Nederlandse apotheek (BrE: chemist’s).
Het is een kruising van de apotheek en de Nederlandse drogisterij als Etos, Trekpleister of Kruidvat.
Vaak kunnen er ook foto’s worden afgedrukt (“1 hr photo service“), kan men er 24 uur per dag terecht (“24-hour service“) en kun je er een griepprik halen of andere preventieve inentingen. Er zijn ook “drive-thu pharmacies”.
Grote Amerikaanse pharmacy ketens zijn Walgreens en CVS Pharmacy.

Voorbeeld (IntE formerly) apothecary (= ‘apotheek oude stijl’) in een AmE tekst:

(AmE) funeral parlor’s limousine en (AmE) mortuary limousine komen als synoniemen o.a voor in het boek The Burden of Proof van Scott Turow (chapter 2). In de roman Bandits van de Amerikaanse auteur Elmore Leonard wordt (IntE) hearse gebruikt.

13c. casket/coffin
In America, and in some other English/speaking countries outside Britain, undertakers tend to use casket rather than coffin. In newspaper accounts of funerals in these countries, the words casket and coffin are often used interchangeably in the same column. In Britain,the normal word is coffin, and casket is normally reserved for ‘a small, and often ornamental, box or chest for jewels, etc.’, and sometimes ‘a small wooden box for cremated ashes’.” (From Fowler’s, 2004)

Voorbeeld van een dialoog bij een Amerikaanse begrafenisondernemer (voorkeur voor ‘casket‘):
The old man, Mr. Bertoia, said to Vincent, no, it didn’t have to be closed. (…) Friends, loved ones, they want to see the departed, they don’t want to look at the coffin. The younger Mr. Bertoia said it wasn’t a coffin, it was a casket. (…) The son was balding, sallow; he stood with his hands behind him in the pose of a minor official, always right (…). Het spreekwoord “een nagel aan de doodskist” = “a nail in the coffin” in zowel BrE als AmE
Voorbeeld:

AmE voorbeeldzinnen van zowel het meest voorkomende ‘casket‘ als het ook regelmatig voorkomende ‘coffin’, soms ook naast elkaar als synoniemen.
– (…) a solid bronze casket no more than four feet in length. Vanessa gawks at it in horor, as if she had stumbled upon a crime scene and found some poor child’s body. (The Racketeer, John Grisham, chapter 36)
– He very nearly came home in a coffin. (King and Maxwell, David Baldacci, 2013, ch. 14)
– His coffin rests on a rolling gurney (…). (…) The palbearers slowly roll the coffin down the iasle (…). The casket rolls by (…). (Steve Martini, The Arraignment, chapter 5)

Het gebruik van ‘casket’ als synoniem van ‘coffin’ (in de betekenis van ‘doodskist’) zelden in het Brits-Engels. Frances Fyfield is een Briys schrijfster die een uitzondering op de regel, bijvoorbeeld in de roman Seeking Sanctuary van Frances Fyfield (2003):
“[T]he coffin was lowered and water sprinkled upon it. (…) Anna had the urge to fling her piece of earth over the crowd instead of letting it drop on the casket.” (Frances Fyfield, Seeking Sanctuary, chapter 1)

13d. undertaker / mortician / funeral director
mortician is an American genteelism (..) for an undertaker (..) or funeral director.” (From Fowler’s, 2004)

In het Amerikaans-Engels worden deze drie woorden als synoniemen willekeurig door elkaar gebruikt. Zie bijvoorbeeld  Scott Turow’s roman Ordinary Heroes (pagina 1) en Elmore Leonard’s Bandits (chapters 1&2).
Voorbeelden (AmE) mortician als alternatief voor undertaker en funeral director:
– Leo was the last of Mullen & Sons, Funeral Directors (…) (Elmore Leonard, Bandits, chapter 1)
– Jack said to him one time at Mandina’s, “You should never have been an undertaker.“And Leo said, “Now you tell me.” (Elmore Leonard, Bandits, chapter 1)

In het Brits-Engels is de term mortician niet gebruikelijk.

14. premenstrual syndrom (PMS) / premenstrual tension (PMT) = (Ned) premenstruaal syndroom
Het Amerikaans medisch standaardwerk “The Merck Manual of Medical Information” gebruikt de term PMS (premenstrual syndrom).
Volgens het OALD (7e en 8e editie, 2005 en 2010) is premenstrual syndrom (PMS) de Brits-Engelse term, en wordt de alternatieve term ‘premenstrual tension’ / PMTzowel in het Brits (BrE) als in het Amerikaans-Engels (NAmE) gebruikt.
In de 9e editie van de OALD wordt PMS aangeduid als de algemene term (BrE/AmE), en PMT als exclusief BrE.

Het Amerikaanse “The Merck Manual of Medical Information” maakt voor de schimmelziekte candidiasis onderscheid tussen o.a. een schimmelziektes (fungal infection) aan het mondslijmvlies = thrush (= Ned. ‘spruw’, ‘candiasis’)
en de vaginale variant = yeast infection

In sommige, vooral archaísche teksten worden de woorden in verswchillende betekenissen gebruikt.
“Barber”duidt dan een barbier aan die zich bezig houdt met het scheren van baarden.
“Hairdresser”is de persoon die de haren en kapsels van mensen verzorgd en modeleerd.
In de reisgids “Ebel’s Switzerland” (1820) schreef de auteur Daniel Wall dat “in Switzerland the barbers are not allowed to dress hair, or hair-dressers to shave”: “A traveller who dines and sups at the table d’hôte, keeps a valet de place, gets shaved, and sends for a hairdresser, must reckon his daily expenses at six florins”(p. 29).

In de betekenis van (rijdende) brancard(wagen) wordt “gurney” meestal met het Amerikaans-Engels geassocieerd, en “stretcher” met het Brits-Engels.
De volgende voorbeelden laten zien dat de woorden in beide taalvarianten voorkomen.
Alleen het woord “(stretcher) trolley” is exclusief Brits-Engels.

31. ventilator / respirator / artificial respiration/ artificial ventilation
(AmE/BrE) ventilator = beademingsapparaat
(AmE / BrE) respirator = ademmasker, adembeschermmasker, stofmasker, mondmasker. Respirator,  wordt specifiek gebruikt voor een bepaald type ‘surgical mask‘, met name van het “N95” type (het getal ’95’ verwijst nar het percentage deeltjes dat uit de lucht gefilterd wordt). Er zijn ‘respirators’ die alleen de omgevingslucht filteren, maar er zijn ook ‘respirators’ die bestaan uit een masker aangesloten op een afgesloten reservoir met zuivere lucht. De respirator is een vorm van PPE (Personal Protection Equipment)

vertalerstips en dilemma’s
In het dagelijks spraakgebruik wordt, zo bleek uit het gebruik van de term in de Amerikaanse media tijdens de coronacrisis in 2020, hebben de termen ‘respirator’ en ‘ventilator’ een verschillende betekenis. De term respirator wordt uitsluitend gebruikt voor ‘gezichtsmasker‘, terwijl een ‘beademingsapparaat’ steevast met ventilator wordt aangeduid. Dit onderscheid is sindsdien in de internationale Engelstalige media algemeen gebruik geworden.
Het onderscheid tussen ‘respirator’ en ‘ventilator’  wordt echter niet altijd gemaakt. Volgens de Engelstalige woorden boeken van Oxford en Webster zijn de termen respirator en ventilator in de betekenis van ‘beademingsapparaat’ synoniem.The Shorter Oxford English Dictionary (5e editie) definieert zowel ventilator als respirator als : (3) (medicine) an apparatus for artificially maintaining respirationWebster’s Unabridged Dictionary (2e editie uitgegeven door Random House) beschrijft respirator als: (medicine) an apparatus to produce artificial respiration. En bij het lemma ventilator vermeldt dit woordenboek: ‘a respirator’.

De synonieme betekenis van ventilator en respirator, respectievelijk artificial respiration / artificial ventilation blijkt ook uit het onderling inwisselbare gebruik van deze twee termen in de volgende voorbeeldzinnen:

Daarnaast heeft de term  respirator in het medisch jargon ook nog de tweede betekenis van ‘ademmasker’/ ‘adembeschermmasker’ / ‘mondmasker’.
The Shorter Oxford English Dictionary (5e editie) definieert respirator in deze betekenis als volgt:  (2) a filtering device covering the mouth …. Hierbij wordt ook de verouderde betekenis vermeld: (formerly) a small mask of gauze or other material intended to warm inhaled air.  En het Amerikaans-georiënteerde Webster’s Unabridged Dictionary definieert deze betekenis van respirator als volgt: (1) a masklike device, usually of gauze ….  (2) see ‘gas mask’   Betekenis (1) wordt in dit woordenboek niet met de aanduiding formerly (“voorheen”) gemarkeerd zoals dat in The Oxford Dictionary het geval is.

De in Nederland uitgegeven tweetalige woordenboeken geven verschillende en tegenstrijdige vertalingen voor ‘beademingsapparaat’/ ‘beademingstoestel‘:
Van Dale’s woordenboek (4e editie) geeft als vertaling voor ‘beademingstoestel’  alleen respirator. Dit is onvolledig en niet actueel.
– Van den End geeft in het Juridisch Lexicon (5e editie) uitsluitend ventilator als vertaling voor ‘beademingsapparaat’, terwijl hij om onduidelijke redenen bij ‘beademingsapparatuur’ de vertaling artificial respiration equipment geeft.
– Het Woordenboek Geneeskunde van Kerkhof (4e druk) geeft, net als de Oxford Dictionary en Webster’s, zowel respirator als ventilator in de betekenis van ‘beademingsapparaat’.
In het Engels-Nederlandse deel van het woordenboek wordt echter bij ‘respirator’ niet de tweede betekenis ( ‘ademmasker’/ ‘adembeschermmasker’ / ‘mondmasker’) gegeven.
– Het Verpleegkundig Woordenboek van Elsevier (2007) geeft voor ‘beademingsapparatuur’ niet alleen de vertalingen respirator / ventilator, en life support system, maar vermeldt als vertaling voor respirator ook ‘ademhalingsapparaat (ademmasker)’.
– Het Medisch Woordenboek van Mostert (5e editie) geeft voor ‘beademingsapparatuur’ de vertalingen respirator, ventilator, en life support system, en vermeldt als vertaling voor respirator daarnaast ook ‘adembeschermmasker’. 

De website van Meriam Webster geeft de volgende historische verklaring voor het feit dat ‘respirator’ nog steeds in de woordenboeken wordt vermeld als synoniem van ‘ventilator’:
However (…), the second definition we give for respirator is “a device for maintaining artificial respiration; ventilator (sense b).” Why would we do such a thing? Because people have been using respirator to mean ventilator for a very long time. The ‘mask’ sense of respirator is older, dating to the first half of the 19th century, but the ‘device’ (or ‘ventilator’) sense of this word is almost as old, in use since the 1850s. It can be confusing, as both words are concerned with breathing, or respiratory functions, and there is quite a bit of semantic overlap. If you are concerned about misuse simply restrict your use of respirator to the mask that protects the wearer (and others), and that of ventilator to the device that assists in breathing.
